A sworn declaration is a document which sets out a number of facts on paper to which you will swear to its truth in front of a notary public / commissioner of oaths.

Although the notary is not required to review the contents of the document, he or she will want to confirm your name and identification is the same as that contained on the document. Once confirmed, the notary will administer the oath and you may sign.
